Enrollment Forms
If you have not already done so for the 2011-2012 school year, please complete a Colonial Kids Annual Enrollment Form which includes a Medical and Media Release. For the safety and security of your child, this form must be completed prior to your child's participation in any Colonial Kids programming/activity.
